What Is BMI and How Is It Calculated?

Body Mass Index (BMI) is a numerical value calculated from a person's height and weight that provides a standardised screening measure for body fatness. It was developed by Belgian mathematician Adolphe Quetelet in the 19th century and remains the most widely used population-level health screening tool globally. The BMI Formula

BMI is calculated by dividing weight in kilograms by the square of height in metres:

BMI = weight (kg) ÷ height² (m²)

In imperial units: BMI = (weight in lb × 703) ÷ height² (in²)

For example, a person who weighs 75 kg and stands 1.75 m tall has a BMI of 75 ÷ (1.75 × 1.75) = 75 ÷ 3.0625 = 24.5 — placing them squarely in the Normal range.

BMI Classification Categories (WHO Standard)

  • Severe Underweight — BMI below 16: Associated with malnutrition, eating disorders, or serious illness. Medical attention is recommended.
  • Underweight — BMI 16 to 18.4: May indicate inadequate nutrition or underlying health conditions. Gaining weight through improved nutrition and exercise is typically advised.
  • Normal Weight — BMI 18.5 to 24.9: The range associated with the lowest risk of weight-related health complications. This is the target range for most adults.
  • Overweight — BMI 25 to 29.9: Indicates excess body weight. The risk of conditions like type 2 diabetes, hypertension, and cardiovascular disease begins to increase in this range.
  • Obese Class I — BMI 30 to 34.9: Moderate obesity with significantly elevated health risks.
  • Obese Class II — BMI 35 to 39.9: Severe obesity with high risk of serious complications.
  • Obese Class III — BMI 40 and above: Also called morbid obesity. Associated with very high risk of life-threatening conditions including heart disease, stroke, and sleep apnoea.

Limitations of BMI — What It Does Not Measure

While BMI is a useful and convenient screening tool, it has well-documented limitations:

  • Does not distinguish fat from muscle: Athletes and bodybuilders often have a high BMI due to muscle mass, not excess fat. A professional rugby player may have a BMI of 28 while being extremely fit.
  • Does not account for fat distribution: Where fat is stored matters significantly. Visceral fat (around the abdomen) carries much higher health risks than subcutaneous fat (under the skin). Two people with the same BMI can have very different risk profiles depending on fat distribution.
  • Age and sex differences: The same BMI value carries different health implications for men and women, and for different age groups. Older adults naturally have more body fat at the same BMI as younger adults.
  • Ethnic variations: Research has shown that people of Asian descent face elevated health risks at lower BMI thresholds. Some health guidelines use a BMI of 23 (rather than 25) as the overweight threshold for Asian populations.
  • Pregnant and breastfeeding women: BMI is not an appropriate measure during pregnancy.

Healthy Weight Range Calculation

BMI for Children and Teenagers

The standard adult BMI categories do not apply to children and teenagers under 18. For younger people, BMI-for-age percentile charts are used instead, comparing the child's BMI against other children of the same age and sex. A child is classified as overweight if their BMI is at or above the 85th percentile and obese at or above the 95th percentile.

Beyond BMI — Other Body Composition Measures

  • Waist circumference: A waist measurement above 88 cm (35 in) for women or 102 cm (40 in) for men indicates increased abdominal fat and cardiovascular risk, regardless of BMI.
  • Waist-to-height ratio: A ratio below 0.5 (waist circumference less than half your height) is associated with reduced cardiometabolic risk.
  • Body fat percentage: Measured with DEXA scans, hydrostatic weighing, or bioelectrical impedance. More accurate than BMI for body composition assessment.
